ST. CLOUD -- Stearns County is creating a five-year Health Improvement Plan, and they're using an area-wide survey of 12,000 households to guide them in developing the priorities they should address.

Renee Frauendienst is the county's Public Health Division Director. She says they'll be creating some strategies and action steps for the community to consider.

The survey was conducted in the five-county area and consisted of 44 questions. The questions centered on things like access to health care, nutrition, exercise, financial stress and mental illness.
