Light the Legacy: Advance Health Care Directives

Lynn is the Project Coordinator for Light the Legacy. She holds a degree in Interdepartmental Speech Communications and is certified through Respecting Choices® as an advance care planning facilitator and instructor for advance care planning facilitators. Lynn is an experienced speaker and is passionate about empowering people to discuss end-of-life issues, motivating them to complete health care directives, and generating this momentum in the central Minnesota community.

Light The Legacy is a community-based organization dedicated to improving end-of-life care in Central Minnesota. They hope to inspire ongoing advance care planning conversations among family members and friends, between patients and health care providers, and in the community at large to encourage the people of central Minnesota to make informed health care decisions and to implement programs that facilitate patient- and family-centered end-of-life care. Light the Legacy works to positively impacting the quality of health care in the community by implementing comprehensive programs to facilitate patient- and family-centered end-of-life care, promoting informed decision-making, and encouraging conversations related to health care directives, hospice care, and palliative care.

 

Participants Will:

  • Learn about various medical terms and end-of-life treatment options.
  • Receive information and resources.
  • Be able to complete a health care directive, the document that outlines the health care values and desires for treatment expressed during the conversation.
  • Receive assistance with the development and implementation of systems that support advance care planning and receive performance reviews to improve the quality of systems currently in use.

 

Please email CAMHI@co.benton.mn.us with any specific questions on end-of-life care, prior to the training day, and the present will be sure to address them during the training.

*A Notary Public will be onsite for those who wish to finalize their health care directive on this day. Should you wish to use this service, you must bring a valid photo ID. *

 

Please join us for this training! This training is intended for those living with mental health issues, providers, family members, and community members.
