UNDATED -- You may be receiving a survey in the mail that asks you questions about health and wellness in your household.

Some 12,000 surveys will be mailed to residents in the five central Minnesota counties.

Local public health agencies and the CentraCare Health System have contracted with the Wilder Research firm in St. Paul to conduct the questionnaire.

It will ask you questions about access to health care, nutrition, physical activity, tobacco use and other social, physical and mental health-related issues.

The results will be kept confidential. The findings will be used to establish health promotion priorities at the local and regional level.
